I only use it where 1) or 2) would be appropriate. I have never said it ala 3).
In fact, I challenge anyone to say that my initial greeting is not one of the below:
1) Hey peeps
2) Yo!
I belive 'hey losers' is one of yours.....
I hear it mostly as synonym for 'that's cool'
The greeting form is, in the immortal words of Cameo, 'word up'
